The one ingredient whose published work is in the laboratory, not in people.

Horny goat weed is the common name for Epimedium, and the panel names the species Epimedium sagittatum. In a 2022 study, researchers in Shanghai isolated six of its 8-isopentenyl flavonoids and tested each against PDE5A, an enzyme that controls smooth-muscle relaxation. Three of them, icariin, 2-O’’-rhamnosylicaridide II and baohuoside I, inhibited it, and raised cGMP in rat smooth-muscle cells.

That is cell and test-tube work. It explains why the herb is in formulas of this kind, and it is the reason the side-effects page mentions it: PDE5 is the same enzyme some prescription medicines act on.
